I was trying to understand the reply- it seems Gerry's paper showed no benfit for chemotherapy to reduce metastatic disease

Re: Anal sac adenocarcinoma - chemotherapy

Yes!
I have been in email contact with Jane Dobson at Cambridge- she has been very helpful. There seems to be some exciting data for the use of Palladia in ASAGAC and she kindly sent me this paper:www.vetsurgeon.org/.../4338.Palladia-and-AG-tumours.pdf

Would recommend reading it- Cambridge oncology department currently use Palladia as a single agent (+/- NSAIDs as necessary) at a dose of 2.5 - 2.75mg/kg EOD (lower than data sheet)

I'm afraid he has retired, but you can contact the oncology department more generally via oncology@vet.cam.ac.uk

He is an old dog and when he presented the lump was already about 5cm diameter and on x-ray we could see what looked like a couple of very large lymph nodes. After being given all of the information the owner didn't want to go for referral. So we have been monitoring and controlling symptoms since. At first the calcium levels were normal but at subsequent checks calcium levels had gone above normal. So he is now on prednisolone and hills wd which seem to have reuced the calcium levels. He is also on lactulose to help with defecation. Anyway my point is that he is doing really well and leads a happy life, the owner is fully aware of the situation and is really pleased that he is still a happy dog and they have had an extra 6 months with him so far.

Re: Anal sac adenocarcinoma - chemotherapy

I think faecal incontinence is rather over egged. The dog in question is also 13 years 6 months old- which adds to owner reluctance for surgery, when asked the question would I have surgery if was my old girl I'm afraid I have to say likely not. Concerned about incontinance post surgery with very large mass on the RHS and a surgery on LHS also required.

Re: Anal sac adenocarcinoma - chemotherapy

[quote user="ashvetenry"]I currently have a case (a receptionist's dog) with a large 1ry mass (and a smaller mass contralterally- both confirmed as adenocarc with FNA/cytology) which whilst not causing quality of life threatening signs currnetly, is much too large in my opinion to be resected.
If there isn't metastatic spread then I'd suggest surgery would really be the best option as could be curative - if difficult would referral to a specialist surgeon be possible? Even with spread to the lumbar LNs then surgery can still be an option - have a case that had the primary and met removed, 1yr chemo and now seems to be in long term remission (hesitant to say cured...).

Re: Anal sac adenocarcinoma - chemotherapy

I have had 2 cases treated with carboplatin, but this was follow up chemo after removal of the anal sac tumour (and one of the dogs also had sub lumbar lymph nodes removed due to metastasis). Both dogs did well with no obvious side effects.
