Age/breed? Any more info? Guess where you go next depends on what resources and finances you have to play with. I would be reaching for the ultrasound if possible as well as your faecal tests, and ideally gut biopsies if the ultrasound doesn't show anything obvious. I know with cats 'triaditis' is well recognised, but am not sure if this is the case in dogs too? (ie pancreatitis/ibd/hepatitis combined inflammatory disease). I would be wanting to take a really close look at the pancreas and remainder of abdomen for any indications of neoplasia.

help me please. I saw a text book polyphagic, chronic diarrhea, rack of bones that came back with an immeasureably high, yep, TLI, and a cspli of 380. where to go? would he ever regain conditon on a lifetime of ultra lo fat? will of course check for faecal stuff.

Elanco companion health make tylan powder for chickens etc. The soluble powder contains 100% active ingredient ie 5g of powder equals 5g of active ingredient .

We have a 7.5kg Cairn (was 9kg until is got ill!) with confirmed IBD via biopsy who is now doing a lot better on 5mg Prednisolone SID.

I lost one of my own dogs to IBD - subclinical so I was unaware of it, until she suddenly decompensated into an acute PLE due to Giardia, so I'd second checking faeces sample for any exacerbating cause. Cobalamin supplementation is not curative - it is just replacing what is not being absorbed due to the gut problems, so would likely be long term (weekly 8wks then fortnightly then monthly). Also agree pred dose sounds too low - for IBD it is usually immunosuppressive doses. Metronidazole helps control SIBO and also has immunomodulatory actions. Other drugs I've known tried for IBD include budesonide (gut-active steroid so fewer side effects but very very expenisve), chlorambucil, ciclosporin.

Also, have been having some success with the prokolin enterogenic sachets - come in a box of 30 for about thirty quid to client.

I'm a big fan of testing pancreas function in some of these also, although in many the poor absorption can make the enhanced digestive function slightly redundant.

When you say TLI/B12/Folate pointed towards IBD do you mean they were low,high or normal? If TLI was low, you might want to add a pancreatic enzyme supplement. Some dogs also need ongoing B12 supplementation.

Unsure if you mean she has been on metronidazole daily since diagnosis or if she had a course- if only a course, then long term ongoing metronidazole may help (I usually use 10mg/kg sid/bid, but there is quite a range) or try oxytretracycline

1mg pred seems like a very low dose, so feel you have plenty of scope to increase the pred; I wouldn't consider other immunosuppressives at this stage although something to consider if higher doses of pred don't work.

Probiotics won't hurt.

(Also worth noting that there can be a grey zone between IBD and neoplastic changes so if she continues to be a non responder, might have to reevaluate and poss repeat biopsies)

How long and what dose was the metronidazole? Sometimes if they haven't been wormed for a while you can have a nasty parasite load. Fenbendazole for 10 days or so can be helpful, especially if it's more than just giardia rumbling around in there!

Other medications you could add would be Tylosin, Azathioprin or Ciclosporin. Did you inject B12?

Good luck, they are always kind of frustrating to treat in my opinion
