I would be reaching for the ultrasound if possible as well as your faecal tests, and ideally gut biopsies if the ultrasound doesn&amp;#39;t show anything obvious. I know with cats &amp;#39;triaditis&amp;#39; is well recognised, but am not sure if this is the case in dogs too? (ie pancreatitis/ibd/hepatitis combined inflammatory disease). &lt;p&gt;Elanco companion health make tylan powder for chickens etc.  The soluble powder contains 100% active ingredient ie 5g of powder equals 5g of active ingredient .&lt;/p&gt;

Cobalamin supplementation is not curative - it is just replacing what is not being absorbed due to the gut problems, so would likely be long term (weekly 8wks then fortnightly then monthly). Also agree pred dose sounds too low - for IBD it is usually immunosuppressive doses. Metronidazole helps control SIBO and also has immunomodulatory actions. If TLI was low, you might want to add a pancreatic enzyme supplement. Some dogs also need ongoing B12 supplementation.&lt;/p&gt;
&lt;p&gt;Unsure if you mean she has been on metronidazole daily since diagnosis or if she had a course- if only a course, then long term ongoing metronidazole may help (I usually use 10mg/kg sid/bid, but there is quite a range) or try oxytretracycline&lt;/p&gt;
&lt;p&gt;1mg pred seems like a very low dose, so feel you have plenty of scope to increase the pred; I wouldn&amp;#39;t consider other immunosuppressives at this stage although something to consider if higher doses of pred don&amp;#39;t work.&lt;/p&gt;
&lt;p&gt;Probiotics won&amp;#39;t hurt. &lt;/p&gt;
&lt;p&gt;(Also worth noting that there can be&amp;nbsp;a grey zone between IBD and neoplastic changes so if she continues to be a non responder, might have to reevaluate and poss repeat biopsies)&lt;/p&gt;

Sometimes if they haven&amp;#39;t been wormed for a while you can have a nasty parasite load.
