&lt;p&gt;Any very good reason not to spay? This dinosaur would spay, unless the uterus looked fine, and look around while in there.&lt;/p&gt;
&lt;p&gt;Actually, I&amp;#39;d try to talk the owner into a spay anyway or, when the pyo developed next year, the owner would ask why I didn&amp;#39;t last year when....&lt;/p&gt;

I presume some bitches will bleed for longer than the usual 10-14 days? &lt;p&gt;What is the source of the bleeding? &amp;nbsp;Vagina, cervix or uterus? Have you, to put it crudely, put your finger up the vagina? Is there a lump?&lt;/p&gt;
&lt;p&gt;Is the blood loss significant? Do a quick bleeding time test.&lt;/p&gt;
&lt;p&gt;Abdominal ultrasound? Radiography?&lt;/p&gt;
&lt;p&gt;I&amp;#39;d risk a small wager that whatever the proper diagnosis, ovarohysterectomy is going to be helpful or curative. &lt;p&gt;Vaginal swab cytology can help with determining what stage in the cycle she is at and what type of discharge you are dealing with - but if we are assuming it&amp;#39;s stright forward bleeding:&lt;/p&gt;
&lt;p&gt;Ultrasound of the the bladder and uterus useful &lt;/p&gt;
&lt;p&gt;Urine sample + bloods. &lt;/p&gt;
&lt;p&gt;Possibly radiographs of the abdomen.&lt;/p&gt;
&lt;p&gt;If all else is normal then outside chance could be clotting issue (uncommon I think). &lt;/p&gt;
&lt;p&gt;The last case like this that I had turned out to have a tumour of the uterus (found on imaging), so we spayed her. Owner declined histopath.
