If they won't xray, stick it on preds and panacur as will do lungworm and if there's a vasculitis preds should also help.

Failing this, put it on some really expensive and 'really strong' antibiotics - this will likely not cure the dog but will either highlight the error of the owners' ways to them or force them to go to another practice.

Or refer. To a homeopath. They don't need xrays.

I'd certainly do that before a GA just for completeness, other than wait and see if they won't let you do anything else.

If the client ties your hands there is little you can do but i would stop the heart meds as they seem inappropriate.

Not your fault the aggressive dog can't be examined properly without sedation. Guessing at treatment because people don't want to do diagnostics never goes brilliantly in my experience. Can't really treat if you don't know what you're treating, plus you've tried that twice to no effect - pointing that out to the owners may show them the need to do more to diagnose.

Re: It looked like caridac disease, now I'm not so sure!

Ehrlichiosis could cause vasculitis, dyspnea and neutropenia.
Is it possible to take the dog's temperature? Has it ever travelled abroad?

At the end of the day what are you supposed to do if the needs to be knocked out to examine it properly and the owner's wont let you? Frustrating.
