&lt;p&gt;sometimes that&amp;#39;s the best you can get&lt;br /&gt;I had a case that went to a number of referral centres and had every test and treatment, even had a unilateral turbinectomy, but remained snotty forever&lt;/p&gt;
&lt;p&gt;I have heard of people using itraconazole longterm in LP rhinitis cases (that don&amp;#39;t have primary fungal rhinitis), though I forget the exact rationale&lt;/p&gt;

If so then there is a very high chance this is aspergillus. I had a similar case in a rottie a few years back and everything came back negative. After much discussion with various specialists we went ahead without a confirmation and drilled into the frontal sinuses on each side and packed full of Canestan cream. Much improvement on round one, repeated a further two times some weeks later as discharge gradually increasing and the dog was then well in remission after this. &lt;p&gt;Possibly you could trephine the frontal sinuses and use them as depot sites to trickle medication, by filling them with a topical gel or cream? With the owner&amp;#39;s full understanding, of course.&lt;/p&gt;
&lt;p&gt;I did once solve what seemed to be an intractable problem by introducing Otomax via the sinus. I&amp;#39;m not suggesting that would necessarily be a good thing to do (and the case was rather different, the problem having arisen originally from damage by a foreign body) but it can be done.&amp;nbsp;&lt;/p&gt;
&lt;p&gt;And, er, teaching granny to suck eggs I know, but nasal biopsies can be misleading.&lt;/p&gt;

&lt;p&gt;radiography normal on all views, endoscopy - red swollen mucosa biopsy just inflammatory reaction, +++ thick mucous in nasopharynx , serology negative for aspergillosis , culturesall been negative fungal and bacterial. &lt;/p&gt;

&lt;p&gt;Sinus trephining is easy and dogs tolerate bits of rubber tube stuck out of their foreheads very well. &lt;div style="CLEAR:both;"&gt;yes to all these , and also to immunocomprimising doses of steroids, various ab&amp;#39;s have been used whenever the discharge has become green or purulent looking but even then nothing has been cultured. &lt;/div&gt;

Maybe immune mediated, could try some corticsteroids but make sure its not Aspergillus first. Sinus trefining sounds worse than the problem. We have a minature Dachshund with this problem, its been to the RVC and had every test under the sun and&amp;nbsp;eliminated&amp;nbsp;aspergillus as far as possible, trefining was not mentioned as a possibility. Bisolvon and antibiotics (Ronaxan) was the only thing to give it some relief, maybe the doxocyclin has some immunomodulatory effect.&lt;/p&gt;&lt;div style="clear:both;"&gt;&lt;/div&gt;</description></item></channel></rss>
