I use 0.5 mg/kg BID after hearing from a few referral people that all they do with a lot of their rabbit cases from first opinion practices is increase the metacam (if at the 0.3 dose, or only sid) and see improvements. I've seen this in cases in practice too. Seems good pain relief helps-who'd have thought it!

So did I, but it was recommended at a CPD lecture on chronic pain that I went to last year. Haven't tried it yet though. Pardale's also supposed to be useful for cases with renal insufficiency, where it's apparently safer than nsaids

It is contraindicated in the datasheet although I did discuss it with the manufacturers once who accepted that the data sheet was written around 20 years ago and has probably not been reviewed for years. Using it in combination follows a discussion with the owner about quality of life vs possible side effects but I find the combination can be quite effective. I use it at a paracetemol dose rate of 10mg/kg rather than the data sheet amount of around 3 times that.

In human medicine paracetemol is a mainstay of analgesia and, as you say, used in combination with NSAIDs and has been reclassified in a group of its own. It is fairly non-irritant to the stomach and relatively safe with renal impairment. The codine component is fairly small and does not appear to be associated with and particular side effects.

As a precaution we have "Do NOT give to cats" printed on the dispensing labels.
I think paracetamol has been reclassified in human medicine and I know they use it alongside conventional NSAIDs all the time, but I have limited experience of using it in veterinary medicine - mainly overdoses where clients used it without veterinary advice in small dogs (and cats - goes without saying), which gave me a fright for life. And, although we all know cats are not small dogs, clients do treat everything as (not so small) humans and often dose them (cats, dogs, etc) the same.
Ketamine in low doses is well recognised as an extremely good adjunct in a multimodal analgesic approach. From Trocoxyl propoganda, central sensitisation is foremost in most of our minds. I do not see the criticism - I found it a very useful contribution and I would like to try it. For one, I think ketamine has infinitely fewer side effects in our patients (low dose, occasional use as recommended) than long term high dose nsaids in an older animal.

Another thing - I've seen (and used in patients myself occasionally) ketamine used on its own at 5mg/kg im in fractious cats to take blood samples - it does not seem to elicit a strong reaction on injection without the dom/torb addition and provides enough time to take samples and clip nails (old cats) without dropping the BP. [I have seen a hyperthyroid cat drop dead after getting extremely worked up during restraint for conscious b/s].

I find ketamine a very useful drug in veterinary medicine (perhaps because I haven't yet experienced the effects personally?)

Re: Okay, so which NSAID is best?

and it was cheap.

Re: Okay, so which NSAID is best?

I am a Metacam girl! It is my first line treatment for arthritis in both cats and dogs, I have found it safe, palatable and very effective until, in dogs, things get worse and I add in Tramadol. I particularly like the flexible dosing and have never seen any significant side effects or long term problems, even in cats.

Re: Okay, so which NSAID is best?
Not CRI but I have given 0.5 mg/kg IM on closure.
Ditto (but tend to give it at start of op) and have found it works really well. Also quite good for those occasional bitch spays that starting puffing, despite premed analgesia, when you pull on their ovaries.

Re: Okay, so which NSAID is best?

apparently works by resetting the wind up process

Then again, perhaps it doesn't.
I guess it would respond similarly to Ketamine.

Re: Okay, so which NSAID is best?

apparently works by resetting the wind up process

They did that on the TV series 'House'!

I find this very interesting and will definitely give it a try. I've used ketamine peri-operatively for orthopaedic cases for a while now and I have been very satisfied with the results. Since I started, I thought my cruciate repairs have been getting ahead of my 'rehab schedule' - specifically when I compared one dog where I used it during the 2nd repair (the other leg of course).

I think there was a BSAVA congress lecture a few years back that suggested to use a constant rate infusion of ketamine for bitch spays as well. Has anyone got any experience/thoughts on it? Sorry, I digress.
