I think we delude ourselves if we believe that the majority of our clients follow instructions as given. I'd estimate that at least a third of our patients would get better with or without our intervention - maybe it'd take a bit longer, but they'd get better. I think that estimate is conservative.

So when things get better, it's not necessarily because we've done such a fantastic job of prescribing just the right thing and the owners have followed our instructions to the letter. Chances are, they've not, but the patient got better anyway.

Cynical? Maybe, but I tend to think of it as realism.

I certainly don't encourage irregular dosing of ears with topical medications, but unfortunately we have some long standing clients in the practice who have been allowed to get away with it for a while, and don't take kindly to me refusing to dispense some ear drops to have at home "just in case he needs them" !! We also have a number of those who don't complete a course and so have some in the cupboard to use when completely inappropriate, and also those who happened to be given some by a friend who's dog had some drops for its ears a few months back, "so we've just been trying these first before coming to see you now it's getting worse"..........

We see plenty like this. Unfortunately previous vets in the practice used to give out ear drops freely. I get particularly frustrated with the ones who have been given repeated bottles of aurizon. Then the owners get really annoyed when I tell them they have to come in to be seen. "well so and so used to give me a bottle every time I phoned up and he never made me bring the dog in"
