Does anybody have an idea about the following case, which is really frustrating me.
It seems like nothing special. Just a rash in the groin area of a 8 year old Fox terrier, located mainly on the left side.
I must admit I am not very good at

Re: rash in a fox terrier
Has anyone successfully treated confirmed demodecosis with advocate?
Got a case ongoing at present - so far so good! (but mild disease, not sure I would use it if very generalised or severe).

On the topic of antibiotics - had an owner this week who was complaining her dog's skin was no better despite a 'course of antibiotics'. On further questioning, she had only given one tablet as it "obviously wasn't a skin infection anyway"........

Re: rash in a fox terrier

There is certainly infection here, with or without underlying skin disease.

Choice, dose and duration of antibiotics are critical for management. I would use a cephalosporin at high dose (25 mg/kg q 12h) for 3 weeks, and consider malaseb 3 times weekly in addition. If not responding over that time period then I would biopsy for culture and histopathology, but send these to a good dermatopathologist in order to maximise the information you get.

I like to take skin scapes initially or very early on to exclude ectoparasites, even in cases where Stronghold or Advocate have been used.

I got burnt about 3 years ago; pruritic SBT, costs an issue, seen vets at several practices and given CCS, and because the clients declined any investiagtion I sent it on its way with more preds. Much later on dog was referred where Demodectic mange was diagnosed. Letter of complaint that we "missed it" . Because I was watching the £'s (on behalf of the client) I assumed that it probably was not parasitic as the dog had been on Advocate for 6 months - now always offer/do skin scrapes.

Antibiotic were given 2 weeks.

I think I did scrape it, but I think I will double check and rule out leishmania/ehrlichia too

rule out leishmaniosis.if it doesnt look like it,it could be it.seen over 50 cases last year.

have scrapings and impression smears,to check for bacterial overgrowth syndrome.

if you have any injectable antibiotic such as convenia by pfizer, which lasts for 14 days have the dog injected for at least 2 times after resolution.also use a combination of antibac/antifungal shampoo twice a week and make sure that it stays on for 8 minutes before rinsed.

by inj you make sure that the dog is getting his medication....

also ask them about where the dog lives, sleeps and pees.

some male dogs have it because they wet themselves!!!

Re: rash in a fox terrier

Too true Clive - always put SS's into the diagnostic plan :)

Re: rash in a fox terrier

I like to take skin scapes initially or very early on to exclude ectoparasites, even in cases where Stronghold or Advocate have been used.

I think that often antibiotics are not given for long enough with pyoderma cases. I would definitely want three weeks of cephalexin or similar spectrum antibiotic before more invasive diagnostics.

Re: rash in a fox terrier

First thoughts are definitely pyoderma. If possible try to find out what order things go in ie is the dog pruritic first, then does the redness develop, and then the papules? Or do the papules come first and then the pruritus/erythema? If pruritus comes first, then allergy more likely.

Also which antibiotic has been used, and at what dose and for how long?

Re: rash in a fox terrier

Is it pruritic? Is tha animal scratching - i.e. prutitic usually = allergy (food allery often responds poorly to steroids), self trauma will exacerbate lesions. If not I would think pyoderma with folliculitis/furunculosis. If BS ab's not working for 2-3 weeks at least then I would suggest bx and culture - concentrating on new lesions is best - at least 3 biposies from areas - pref non-ulcerated.
