&lt;p&gt;If it is not responding to simple antibiosis you need to look at other things. Does the dog have underlying skin disease eg atopy? Is there any possibility of a foreign body? Could there be an underlying behavioural issue causing self-trauma?&lt;/p&gt;
&lt;p&gt;Just a few thoughts hope this helps.&lt;/p&gt;
