Feline Diabetic Remission Clinic<o:p></o:p>

The Royal Veterinary College is busy unravelling the riddle for the best treatment of diabetes mellitus in cats. <o:p></o:p>

Dr. Stijn Niessen and his team have officially launched a new Feline Diabetic Remission Clinic in the Queen Mother Hospital for Animals in Hertfordshire, to investigate the optimum methods to achieve good control, and ultimately diabetic remission, in diabetic cats.<o:p></o:p>

Cats examined through the new clinic will benefit from a host of incentives, including free food and insulin, as well as optimum diabetic management, with the support of Internal Medicine Specialists, while also contributing immensely to the current scientific knowledge on the best treatment for diabetic cats.<o:p></o:p>

As we all know, feline diabetes mellitus is a costly, chronic disease which can significantly decrease the quality of life of both cat owners and their pets. However, a proportion of diabetic cats can achieve diabetic remission and no longer require insulin therapy, which is increasingly considered the ultimate goal in treating diabetic cats. <o:p></o:p>

Early evidence suggests that good, early blood glucose control increases remission rates in diabetic cats. However, there is little evidence about which treatment protocol offers the best blood glucose control and chance of remission.<o:p></o:p>

The clinic is currently recruiting cats that have had diabetes for less than 4 months' duration for a clinical trial, examining the effect of new insulins on blood glucose control and remission rate.<o:p></o:p>

Enrolled cats will qualify for full underlying disease assessment at the referral hospital, several re-examination appointments, 12 months of prescription diabetic food, and 6 months of insulin therapy – all at no cost to owners. We are also interested in receiving blood samples from all diabetic cats, regardless of duration of diabetes mellitus. Newly diagnosed cats (diagnosis within the last 6 weeks) are also eligible for follow up samples for 4 months.<o:p></o:p>

Canine inflammatory bowel disease (IBD) in German Shepherd Dogs

Dear SamSoc members,

As part of a PhD project I am currently investigating the genetic background of canine IBD in German Shepherd Dogs (GSDs). In order to perform an adequately powered genomic scan as representative as possible of the UK population of this breed, I am seeking to collect residual whole blood samples (1-2 ml) collected from GSDs during the diagnostic investigation of chronic GI signs which led to a diagnosis (endoscopic or surgical) of chronic enteropathy (food or antibiotic responsive diarrhoea and idiopathic IBD). For the submission of the samples I would provide prepaid envelopes containing clients' consent form, project information form, clinical questionnaire. The project and the recruitment of samples from outside the RVC has received at priori ethical approval. Your help and support will be adequately aknowledged and very much appreciated!

Idiopathic Pulmonary Fibrosis study

Hello all

Cecile Clercx, at the University of Liege, is recruiting cases of IPF for a research project which aims to investigate potential serum biomarkers to facilitate early diagnosis, understand the genetic background, and investigate the mechanisms of disease in the hope of identifying treatment targets. Cecile needs to recruit as many dogs as possible for this study and would be keen to hear from any vets in first opinion or referral practice who think they might have a case. Participation in the project requires some straightforward form-filling, and submission of any available blood and ideally lung samples (can be collected PM) to Liege. There are some specific requirements for sample submission, so if you think you have a case then it is best to contact Cecile in advance and she can send you the information and sample pots.

Winter meeting

Dear members

We are delighted to present the provisional plan and costs for the upcoming winter meeting. Some of the finer details and topics will follow, however the outline is summarised below.

As many of you know we have been making efforts to meet at the end of the year. The aim of this meeting is to try to develop collaborative research between our private practice and university-based members. We have decided that this first meeting will take place on a single day. The outline of the day is shown below. We hope this will appeal to our membership, particularly those of you in first opinion practice who we are particularly keen on being involved in this venture. As we all know there are so many crucial questions relating to cases we see every day that would be great to answer. These questions will be so much easier to answer with collaboration. We hope that with input from our members with expertise from first opinion practice and those active in research we will be able to generate projects that will make significant contributions to patients with common problems.

The outline for the inaugural meeting outline is as follows:

Friday 23^rd November

Registration opens at 6pm – till 8pm

Dinner 8.00-10.30pm – venue TBC

Saturday 24^th November

8.00-9.00am Registration reopens

9.00am Welcome and introduction to the day

9.15am Overview of developing research collaborations with first opinion practice – outlining the importance of first opinion case based research to attempt to address the current bias to referral populations in the literature.

10.00am Coffee (biscuits!) and discussion

10.30am Presentations by Protexin and Royal Canin on the current evidence for the use of probiotics and other neutraceuticals in dogs and cats

11.30am Presentation by the Pet Blood bank on current knowledge and future perspectives on the availability and use of blood products in dogs and cats

12.30am Case report and active research project presentations

1.30pm Lunch and discussion

2.30pm Review of projects proposed on the forum and new project proposals – thoughts and ideas from membership.

New project proposals to be presented in a 5 minute slot using 3 ppt slides or overheads, however visual aids not compulsory.

3.45pm Summary of project discussions from the afternoon – proposals presented to rest of group with decision over which projects to take forwards and who will be involved in these.

4.00pm Coffee (biscuits!) and discussion

4.30pm Closing remarks and feedback

5.30pm Mentoring session for those keen on discussing projects with particular supervisors

The cost for the day has been fixed at £30 which will include lunch and registration fee.

The venue is the School of Veterinary Medicine and Science, University of Nottingham, Sutton Bonington Campus, Leicestershire, LE12 5RD

This is the last year for me (KM) to serve as Chair

· The programme committee (Stijn, Sheena and Mark D) have done a great job and produced a great programme and David and Yvonne for sterling work advertising on listserve etc. Stijn had the job of physically carrying the heavy notes here.

Yvonne particularly has done everyone’s jobs and has streamlined processes by making a task list for each committee role.

Thanks to Mark M for work as treasurer (who would like to step down as he has a new business to take care of. He will stay on if the need arises).

Thanks to Rebecca for sorting ICC it has certainly been more efficient, to Marge Chandler for organising notes even though now off committee, David W for PR –increased awareness a lot though mail shots. Thanks to Allie W secretary who is also due to step down this year – kept going through tricky family times. Also thanks to Allie’s hubby Jason who has previously audited accounts but now due to change in position in his real job will step aside but Allie offered to continue doing the auditing.

Sheena is going to take the lead on programme

We wish to have a new formal committee position to deal with the forum as this has previously been handled by Ian

· Any ideas for next years programme we have been trying to offer a mix of topics – please post any ideas on the on forum

· Website /forum– continue to encourage use of discussion forum, research trial advertising etc Ian Battersby has done some great work and the forum s being used well – documents such as advertising research projects and new features features and minutes can be uploaded into wiki.

· Antibiotic working group ( IB to comment on progress)

· Constitution – no changes recommended unless vote to lose 1st/2nd proposer. It was discussed and there was a majority in favour to stop new members having to be proposed and seconded by SAMSoc members. We will include a sentence in the constitution that allows a veto vote for members who are troublesome. IAB to action

· Membership and finances to be covered by Yvonne and Mark

· Longer term – We wish the society to grow in size and have good meeting and attendance. We need to try to continue relationship with current sponsors, identify new sponsors, maintain case report prize – great input again this year. Good interest in travel scholarship this year.

Despite the meeting being run at a loss (mainly as a result of the expense of catering) we will increase cost of meeting to subsidise this. There was a majority vote for keeping lunch (lunch – 23.80 + VAT/delegate and water is £1.50/delegate)

· Encourage members to consider coming on committee if and when new members needed.

4. Membership report

136 members with 63 at meeting last year?

This year 118 members, 25 were new members and 68 at 2011 meeting.

Please let the membership secretary know if there is any change in email address.

5. Forum Report (Ian Battersby)

120 people are active on the forum, with 200 discussions since forum started. Plea to members not to be afraid to have an opinion – just disagree nicely!

6. Treasurer’s report

In 2010 there appears to be an improved profit, however that is because last years Hill’s sponsorship came in late.

Members registered earlier for the meeting this year which was helpful.

The bank balance total is financially sound.

In 2010 the meeting made a loss of £3000 this was supported by £2000 sponsorship.

Pfizer are committed to provision of £500 to be put towards the sponsorship of the antibiotic committee’s poster.

This is a small committee with a remit to produce empirical guidelines to moderate use of antibacterials/ provide useful suggestions. The aim was for it to be a poster but currently it is a 40 page document. There is planned a meeting with BSAVA to produce a draft later on the day of the satellite meeting. Ian Battersby to maintain updates.

a. Anyone standing down – if so we look to floor for volunteers & thank outgoing person

EPI is a common condition in dogs with early response to therapy being critical for positive outcomes (Batchelor et al, 2007). Pancreatic enzyme supplementation is the key component of therapy in EPI. Numerous enzyme preparations exist for use in dogs, some of which are enteric-coated and some uncoated. Although one retrospective study on treatment of EPI suggested a better response with uncoated preparations (Hall et al., 1991), there was no difference in response amongst preparations in a more recent study (Batchelor et al., 2007). However, prospective clinical trials, on treatment of EPI, are lacking.

The Small Animal Teaching Hospital, University of Liverpool, is performing the first randomised double-blind trial to examine the effect of enteric coating on efficacy of pancreatic enzyme supplementation in the treatment of canine EPI. We are aiming to recruit dogs recently diagnosed with EPI for inclusion in this trial. Enrolled patients will undergo clinical evaluation, two months of treatment and follow-up advice, all free of charge.

Inclusion criteria:

Clinical signs consistent with EPI (weight loss, poor weight gain, diarrhoea, polyphagia, coprophagia
Serum trypsin-like immunoreactivity (TLI) measurement consistent with EPI

Exclusion criteria:

Animals with concurrent diseases that may affect the diagnosis, treatment or prognosis. The decision for exclusion of a particular case will rest with the investigator
Cases that have not had routine haematology and biochemistry analysis performed at the referring veterinary surgeon
Cases with severe disease or debilitation, e.g. requiring hospitalisation and intensive management. However, such individuals may be suitable candidates once recovered
Cases already on therapy for EPI
Owners not willing to travel to the Small Animal Teaching Hospital for 4 consultations during the study period time (2 months)

Study into epileptoid cramping syndrome and Scottie Cramp

I am posting this on behalf of my colleague. Please see details below.

We are seeking from colleagues cases of suspected canine epileptoid cramping syndrome in Border Terriers or of suspected Scottie cramp for the purpose of genetic research. Identification of a DNA marker for these conditions may help us to identify the cause of these disorders, accurately diagnose them and advise breeders in their breeding programs.

A syndrome known as canine epileptoid cramping syndrome (also known as Spike’s disease) has been observed in Border Terriers. Episodes consist of gait abnormalities ranging from ataxia to an inability to stand, contractions of abdominal, neck and back muscles resulting in abnormal posturing and contractions/cramping of the appendicular muscle (extensor rigidity or flexion of the limbs). Duration of the episodes can vary from seconds to half an hour or longer during which the dog remains aware of his surrounding. Increased intestinal motility is suspected based on hearing borborygmus. Affected dogs may experience pain during the episodes.

Scottie cramp is a syndrome observed in young adult Scottish Terriers consisting of involuntary sustained muscle contractions primarily affecting the hind limbs. With excitement, the hind limbs assume a hypertonic, extended position or they may occasionally display exaggerated flexion of the limbs. The forelimbs become abducted and develop increased extensor tone. Affected dogs progressively develop a stiff stilted gait over a few minutes. Severely affected dogs assume an arched posture over their back and may fall into lateral recumbency with their head and tail flexed.

Thanks to our Programme committee Stijn, Sheena, and Marge Chandler. It is difficult to provide a varied and generalised programme that is suitable for both specialists and generalists.

Other members of the committee are very hard working in particular Ian Battersby and Yvonne McGrotty who have performed sterling work on advertising on list serve and setting up the forum with Arlo Guthrie of Vetsurgeon.org.

We would like to to encourage our members to make use of the discussion forum and log on regularly as there are additional features such as the online research trial and job advertisements.

Any members who have not paid their subs by the end of April will be removed from the online forum by Yvonne.

Ian Battersby is invited to discuss the Antibiotic Working Group. See appended section

Constitution

The committee is elected for 3 years and members may be re-elected for a further 3 years.

We extend our thanks to Clive Elwood who is stepping down this year as he resurrected the society and also produced a constitution. We thought that a gift for his dedication was in order so we are sending him for a Tank Driving Experience Day. He has sent an email to thank us for this.

We would also like to thank Marge Chandler for her hard work with the Programme, She will continue to print the meeting notes if required.

We would like to encourage members to consider coming on to the committee when new committee members are needed. Next year we will be needing a new Chair person and Ian Battersby is up to be re-elected next year.

Yvonne will be drawing up a list of the duties of each committee member (as she has worked very hard and has done a little of everyones jobs this year).

In the longer term our aims should be to continue our relationship with our current sponsors., identify new sponsors and maintain the case report prize which had great input this year.

There are 63 members attending this meeting.

Subs are due and should be paid at the time we are paying for the PreBSAVA satellite meeting.

There was majority agreement that it should be paid for all in one sum. Although access to web forum will be restricted to subscribed members only.

Treasurers Report:- Summary

We would like to thank the Treasurer and Dr. Jason Woodward for compiling and auditing the accounts respectively.

In 2009 the society made a profit but it was less than appearences would seem as we had to bank 2 years worth of Hill’ s sponsorship (and they are thanked for that). Therefore the corrected profit for 2009 was somewhat less that the profit in 2008.

The loss for the 2009 meeting was just over £1000 greater than in 2008 but if we take member subscriptions and sponsorship into account we do break even.

The major cost to the society is catering and the cost of the meeting was an extra £800 this year.

We will investigate the cost of a cold stand up buffet to see if catering costs can be reduced.

Obviously bank interest is very low at present so not giving a significant income.

We currently have 144 members and 100 members enrolled on vetsurgeon.org forum

Feedback from 2009 meeting

Largely positive with a mix of comments on a mixture of topics. With trends. Generally the case reports were very positively received. Some people had the opinion that the case reports shortened the time available for the lectures, and whether members would like to increase the number of case reports.

Some members prefer the day to focus on a single topic area others would prefer a mixture. Obviously this variation in opinion cannot be wholly fulfilled and this year we opted for a mixture of talks with a trend in the thromoboembolism theme.

This year the room is hot, whereas last year it was cold!

Human speakers appear to provide shorter summary lecture notes than their veterinary counterparts.

Please would members give feedback on this years meeting .. It may be possible to put a poll on vetsurgeon.org. regarding the interactive case reports.

Travel Scholarship - this should be attractive benefit for membership not advertised well. Another incentive to get on forum and the price of entering for the scholarship is to provide a written piece on the travel undertaken. This report could be published in the veterinary Times would give publicity for the society. It would be interesting to gain insights as to what the value of the travel scholarship should be.

Kate Murphy is reconsidering the membership structure and looking at access to RCVS library - Mark Dunning pointed out that there had been a reduction in the number of journals offered in their archives.

　

ANTIBIOTIC Working Group　

IB:- The Antibiotic Working group has been set up to try to come up with a poster of guidelines for the use of antibiotics in some clinical situations. Progress has been slow initially. It is a large project and we hope to back it up with information. The BSAVA Scientific committee is backing this project and possibly we will produce a back up document. Likely it will be a 2 year project and may not cover all situations. The hope is that general practitioners will use the guidelines and prevent the restrictions to antibiotic use that were described in Sue Dawson
’
s lecture.
The length of the lectures are to be changed to 45 minutes per lecture.

Travel Scholarship - this should be attractive benefit for membership not advertised well. Another incentive to get on forum and the price of entering for the scholarship is to provide a written piece on the travel undertaken. This report could be published in the veterinary Times would give publicity for the society. It would be interesting to gain insights as to what the value of the travel scholarship should be.

Kate Murphy is reconsidering the membership structure and looking at access to RCVS library - Mark Dunning pointed out that there had been a reduction in the number of journals offered in their archives.

　

ANTIBIOTIC Working Group

　

IB:- The Antibiotic Working group has been set up to try to come up with a poster of guidelines for the use of antibiotics in some clinical situations. Progress has been slow initially. It is a large project and we hope to back it up with information. The BSAVA Scientific committee is backing this project and possibly we will produce a back up document. Likely it will be a 2 year project and may not cover all situations. The hope is that general practitioners will use the guidelines and prevent the restrictions to antibiotic use that were described in Sue Dawson
’
s lecture.
The length of the lectures are to be changed to 45 minutes per lecture.

Idiopathic inflammatory bowel disease (IBD) is the most common cause of chronic intestinal disease in dogs. Current treatment protocols most often involve the use of immunosuppressive doses of corticosteroids to reduce intestinal mucosal inflammation and achive remission.

However a number of dogs with IBD treated with corticosteroids have either no response at all to the drug or relapse after weeks to months of treatment. In addition, treatment with prednisolone often results in unacceptable side effects.

Masitinib is a protein tyrosine kinase inhibitor licensed for the use in dogs with mast cell tumours, where it has proven efficacy. There also reports that this drug can decrease inflammation in people with several immune mediated disorders, including IBD.

The Veterinary Clinical Sciences at the Royal Veterinary College is performing a clinical trial to evaluate the clinical efficacy of masitinib as a sole treatment for canine IBD in steroid-refractory cases over a period of 8 weeks.

Inclusion criteria:

- Diagnosis of IBD after excluding other causes of chronic diarrhoea based on routine diagnostic test:

faecal parasitology
faecal bacteriology
CBC, serum biochemistry, urinalysis
abdominal ultrasound
serum TLI
serum cobalamine and folate concentration

- the presence of inflammatory cell infiltrates in mucosal biopsies obtained by gastroduodenoscopy and/or colonoscopy.

- Previous treatment with corticosteroids for at least 4 weeks with only minimal or no response

Exclusion criteria:

- Unwillingness of owner to participate

- Cases that don't meet the inclusion criteria (incomplete diagnostic tests)

Following inclusion in the study all dogs will receive masitinib orally at 12.5mg/kg/day for 8 weeks. At the time of inclusion to the study all cases will be scored with a clinical activity index (CCECAI) and serum biochemistry (including albumin) will be performed and every two weeks thereafter for the duration of the study. The latter test will serve to evaluate clinical efficacy, assess the progress of the disease and also identify the presence of adverse effects of the drug.

Consultation, physical examination, blood work and costs of the drug will be covered by the RVC. Please note that the drug will not be dispensed beyond the duration of the study (8 weeks).

The study has been approved by the RVC Ethics Committee.
